ATTENTION CURRENT PATIENTS!

Orthopedic surgeon David C. Carlson, MD is announcing the closure of his Shakopee, Chaska, and Glencoe offices effective March 1st, 2012.  Dr. Carlson has taken a position with a hospital in Wisconsin.  All patients who were seen within the last year, and for whom we had current contact information on file, were sent a letter which provided this information.  

You may REQUEST YOUR MEDICAL RECORDS by completing the medical release form and faxing to:  952.428.3390.  Please allow 5-7 business days for your request to be fulfilled.

Please note that for medical record requests that include x-rays, an interpretation of any x-rays will be provided in the form of x-ray reports.  it is imperative to obtain the actual films, you may be charged a pre-payment fee based on the number of films to be copied and estimated shipping costs, all of which are done by an outside company. 

Please clearly mark on your release form any requests for x-ray films and allow extra time for this process.
