Insurance Plans

Orthopedic Surgical Consultants, P.A. has contracts with most Health Maintenance Organizations (HMO's) and Preferred Provider Organizations (PPO's).  We accept the following health plans:

As a courtesy, we will submit your claim directly to your insurance company.  If you have a question about your insurance plan, please call our business office at (952) 931-9718 or email business_office@osc-mn.com for assistance.

Some plans require a referral for services with a specialist.  If a referral is required, please contact your primary care physician prior to your appointment.  Under most HMO and PPO contracts, you will be responsible for a co-payment at the time services are rendered
